Blair has his guide voice and his lover, Jim managed to not throw Blair out when Alex showed up, and Naomi turned out to be Alex's real guide.  So, that would seem to suggest that Blair dodged a metaphysical bullet (and a literal drowning).  Unfortunately, the spirit world is not quite ready to let go of Blair Sandburg.
