Spike always plows thought. Before the chip, he had all the subtlety of a bulldozer, although after the chip he wasn't much better. And I would agree that Xander is misinterpreting what's going on because he doesn't know about Cordelia's changed family circumstances, largely because she isn't telling.
And the therapy really is helping Angel and Xander get back on track.
